Job summary

LiveWell Homecare Agency is seeking a Registered Nurse Case Manager to deliver compassionate end-of-life support to patients and families. The role involves assessing needs, developing and implementing individualized care plans with an interprofessional team, and ensuring seamless communication across the continuum of care.

The position requires a Nursing Diploma or Associate’s degree, RN licensure, and at least two years of nursing experience; a Bachelor’s degree is preferred.

Qualifications

• Valid driver’s license and willingness to cover all regions during on-call.
