Goodwin Living in Alexandria, VA, is seeking an experienced Registered Nurse to lead end-of-life care for a patient/family caseload and collaborate with the inter-disciplinary team to tailor plans of care.

Qualifications

• Bachelor’s degree preferred.
• State issued RN license required.
• At least two years of administrative experience, preferably hospice or discharge planning.
• Articulates end-of-life discussions with clients calmly under pressure.
• Proficient in Word, Excel and able to master documentation software to assist others.

Responsibilities

• Assumes primary case management responsibility for a patient/family caseload including initial and ongoing assessments, planning, implementing, and evaluating nursing care.
• Assess patient needs and admit for service when eligible, following protocol.
• Re-evaluate clinical needs at least every 14 days and document eligibility criteria.
• Initiate and coordinate the patient’s plan of care with the IDT per the approved plan.
• Deliver or supervise skilled nursing services using evidence-based end-of-life care.
• Document all visits, med changes, and care plans within 24 hours.
