Goodwin Living, a nonprofit senior living and healthcare organization based in Alexandria, Virginia, is seeking a full-time LPN to deliver end-of-life care and hospice services in the community setting under RN direction. The role centers on planning patient and family care within the interdisciplinary care plan and ensuring skilled nursing care is provided as specified.

Formación

• Graduate of an accredited school of practical nursing.
• Current Virginia LPN license.
• Two years of recent nursing experience in acute/long-term care or home care/hospice; hospice/homecare preferred.

Responsabilidades

• Provide nursing care per Goodwin Palliative Care and Hospice policies and nursing standards.
• Consult with RN Case Manager on changes to the care plan.
• Report significant changes in patient condition to the RNCM.
• Administer medications and treatments as ordered by the physician; pass competency for IV/subcutaneous administration.
• Reinforce interventions, medications and side effects to patient, caregiver, and others as directed.
• Participate in IDG meetings to share information and support.
