Job summary

Ambercare Hospice is seeking a compassionate and dedicated Registered Nurse Case Manager in Espanola, NM to provide expert clinical care to patients and families, ensuring comfort, dignity and quality of life. Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR and First Aid Certification.
• Knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation and evaluation of the nursing process.
• Obtain data from patient’s physical, psychological, social and spiritual factors and initiate interventions by the interdisciplinary team.
• Initiate communication with the attending physician and other agencies to coordinate care.
• Provide regular visits to reassess needs and update the care plan.
• Perform clinical pronouncement of death and support the family.
• Maintain accurate patient records and care plans timely.
• Supervise and document delivery of care by Home Health Aide as required by guidelines.
