Qualifications

• Graduate from an accredited registered nursing program.
• Active RN license in the state of practice.
• Current CPR and First Aid certifications.

Responsibilities

• Lead patient and family caseload across assessment, eligibility, admission, planning, implementation, and evaluation.
• Gather data on physical, psychological, social, and spiritual factors to guide care.
• Coordinate care by communicating with the physician and interdisciplinary team.
• Make regular visits to reassess needs and update the care plan.
• Perform death pronouncement and provide family support.
• Keep accurate records, charts, progress notes, and timely care plans.
• Supervise Home Health Aide delivery of care per guidelines.
