Job summary

Phoenix Home Care in St. Louis, MO is seeking a dedicated Hospice RN Case Manager to deliver patient- and family-centered nursing care.

You will collaborate with an interdisciplinary team to implement plans of care and support with goals of care discussions and healthcare decisions for quality of life. The ideal candidate brings energy, leadership, organization, and strong communication to guide patients and families through challenging health journeys.

Qualifications

• Missouri or Compact RN License required.
• Valid driver’s license and auto liability insurance.
• Hospice experience preferred but not required.

Responsibilities

• Educate patients and families about the plan of care.
• Consult with physicians and Hospice Medical Director on changes to orders or plan of care.
• Coordinate hospice care with the team to meet patient needs.
• Conduct initial and ongoing comprehensive assessments.
• Perform evaluations for hospice care and informational visits.
