Job summary

Phoenix Hospice in Springfield, MO, seeks an RN Case Manager to provide compassionate hospice nursing care to patients and families. You’ll educate patients on the hospice process, support families in goals-of-care discussions, and collaborate with an interdisciplinary team to coordinate care.

The role emphasizes thorough assessments, ongoing visits, and clear communication as part of a mission-driven hospice culture.

Qualifications

• Active Missouri or Compact RN license required.
• Hospice or Long-Term Care nursing experience preferred.
• Strong assessment, communication, and care coordination skills.
• Valid driver’s license and auto liability insurance.

Responsibilities

• Provide compassionate hospice nursing care to patients and families.
• Educate patients and caregivers on hospice process and plan of care.
• Guide families through goals-of-care discussions and healthcare decisions.
• Conduct initial and ongoing comprehensive assessments.
• Perform hospice evaluations and informational visits.
• Collaborate with physicians, Hospice Medical Directors, and interdisciplinary team members.
• Coordinate care to ensure patient and family needs are fully met.
• Communicate changes in patient condition and update plans of care as needed
