Job summary

Phoenix Home Care & Hospice in Jefferson City, MO is seeking a full-time Hospice RN Case Manager to provide compassionate nursing care to patients with terminal diagnoses. You will work with an interdisciplinary team to develop and implement a patient-centered plan of care, guiding goals of care discussions and informed healthcare decisions.

This role offers a Monday–Friday 8am–5pm schedule, with pay range $65,000–$90,000 and a benefits package including medical plans, PTO, 401K, spousal

Qualifications

• Missouri or Compact RN License.
• Hospice or Long-Term Nursing experience.
• Valid driver's license and auto liability insurance.

Responsibilities

• Provide education to patients/family members regarding the process, plan of care, and offers guidance with healthcare decisions.
• Consult with the attending physician and Hospice Medical Director concerning changes in patients condition requiring order changes and/or changes to plan of care.
• Coordinate hospice care by working with other team members as well as with the patients/family members to ensure needs are met.
• Conducts initial as well as on-going comprehensive assessments.
• Preform evaluations for hospice care along with informational visits.
