Phoenix Home Care and Hospice is seeking an RN Case Manager for the St. Louis, MO area.

Role includes providing compassionate nursing care within an interdisciplinary team to develop and implement patient/family centered plans of care, with goals of care discussions and informed healthcare decisions to improve quality of life. The ideal candidate should bring energy, strong managerial and interpersonal skills, and the ability to guide and support others in demanding situations for professional

Qualifications

• Missouri or Compact RN License.

Responsibilities

• Provide education to patients/family regarding the plan of care and guidance with healthcare decisions.
• Consult with the attending physician and Hospice Medical Director on changes in patient condition or plan of care.
• Coordinate hospice care with other team members and patients/family to meet needs.
• Conduct initial and ongoing comprehensive assessments.
• Perform evaluations for hospice care and informational visits.

Job description

• $5,000 Sign on Bonus!
• Full Time: Monday - Friday, Days
• St. Louis, MO and surrounding areas
• Pay Range: $75,000 - $90,000

Phoenix Hospice RN Case Managers provide compassionate nursing care to clients and their families dealing with a terminal diagnosis. Will work with the interdisciplinary team to develop and implement a plan of care that is patient/family centered, with goals of care discussions and guidance with healthcare decisions to give quality of life.

Responsibilities

• Provide education to patients/family members regarding the process, plan of care, and offers guidance with healthcare decisions
• Consult with the attending physician and Hospice Medical Director concerning changes in patients condition requiring order changes and/or changes to plan of care
• Coordinate hospice care by working with other team members as well as with the patients/family members to ensure needs are met
• Conducts initial as well as on-going comprehensive assessments
• Perform evaluations for hospice care along with informational visits

Requirements

• Missouri or Compact RN License
• Valid driver's license and auto liability insurance
