Job summary

Phoenix Home Care & Hospice in Jefferson City, MO is seeking a dedicated Hospice RN Case Manager to deliver compassionate nursing care to patients and families facing terminal diagnoses. You will collaborate with the interdisciplinary team to create and implement patient-centered care plans, guiding healthcare decisions to enhance quality of life.

Responsibilities include education, care plan coordination, and regular assessments, with a Monday–Friday schedule.

Qualifications

• Missouri or Compact RN License required.
• Hospice or Long-Term Nursing experience required.

Responsibilities

• Provide education to patients and family on care plans and healthcare decisions.
• Consult with physicians to adjust plans of care as patient conditions change.
• Coordinate hospice care with the interdisciplinary team and family to meet needs.
• Conduct initial and ongoing comprehensive assessments.
• Perform evaluations for hospice care and provide informational visits.

Phoenix Hospice RN Case Managers provide compassionate nursing care to clients and their families dealing with a terminal diagnosis. Will work with the interdisciplinary team to develop and implement a plan of care that is patient/family centered, with goals of care discussions and guidance with healthcare decisions to give quality of life.

Responsibilities

• Provide education to patients/family members regarding the process, plan of care, and offers guidance with healthcare decisions.
• Consult with the attending physician and Hospice Medical Director concerning changes in patients condition requiring order changes and/or changes to plan of care.
• Coordinate hospice care by working with other team members as well as with the patients/family members to ensure needs are met.
• Conducts initial as well as on-going comprehensive assessments.
• Preform evaluations for hospice care along with informational visits.

Requirements

• Missouri or Compact RN License
• Hospice or Long-Term Nursing experience.
• Valid driver’s license and auto liability insurance.
