You will manage a home health caseload, supervise LPN visits, and collaborate with PT/OT/Speech teams to create personalized care plans for patients with CHF, COPD, diabetes, wounds, and medications. The position offers a base salary of $78,000-$88,000 with a pay-per-visit model and a $10,000 sign-on bonus.

Qualifications

• Missouri RN license or Compact license required.
• Strong organizational, clinical, and communication skills.
• Ability to manage prioritization and time effectively.

Responsibilities

• Manage a caseload of home health patients and supervise visits.
• Coordinate with therapy teams to build personalized care plans.
• Assess, chart, and support patients with CHF, COPD, diabetes, wounds, and medications.
