JourneyCare Hospice in Crystal Lake, IL is seeking a compassionate RN to join our team. You will manage patient and family caseload, assess needs, initiate care plans, and coordinate with the interdisciplinary team to ensure comfort and quality of life.

The role offers weekday hours, one weekend every 12 weeks, and competitive pay with strong benefits, including a 401(k) match and generous time off to support professional growth and personal well-being.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR, First Aid Certification.
• Must be knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance is required.

Responsabilités

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation and evaluation phases of the nursing process.
• Obtain data from patient physical, psychological, social and spiritual factors impacting needs and initiate intervention by the interdisciplinary team.
• Initiate communication with the attending physician and other agencies to coordinate optimal care.
• Provide regular visits to reassess patient needs and update the plan of care.
• Perform clinical pronouncement of death and provide support to the family.
• Maintain accurate patient records, charts, progress notes and care plans timely.
• Supervise and document delivery of care by Home Health Aide as required by guidelines.
