JourneyCare Hospice is seeking dedicated CNAs to join our growing team in Crystal Lake, IL. This full-time role offers weekday hours, competitive pay, and a strong benefits package, including 401(k) match and generous time off.

You will provide compassionate in-home care under RN supervision, focusing on comfort and dignity for our patients. Ideal candidates hold an active CNA license, CPR certification, and a clear state registry record.

Qualifications

• Current certification as a Certified Nurse Aide (CNA) in accordance with applicable state requirements.
• Maintains a current CPR certification.
• List on the state aide registry with a clear record able to deliver care to patients.
• 18 years of age unless state law dictates otherwise.
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy, comfortable providing specialized care to the terminally ill.
• Is self-directing with the ability to work with little direct supervision.

Responsabilités

• Performing personal care needs after given a patient’s assignment and attending to his/her requests promptly.
• Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.)
• Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed lines as often as assigned
• Assisting in dressing and undressing patients as assigned
• Planning and preparing nutritious meals, including shopping, as assigned
• Assisting in feeding the patient as assigned
• Taking and recording vital signs when ordered with competency
• Providing proper care and observation of patient’s skin to prevent tissue breakdown
• Reporting changes to the Case Manager
• Assisting in ambulation and exercise as instructed
• Providing respite for patient/family/caregiver when on-site, as appropriate
• Maintains confidentiality at all times
