Job summary

Tapestry Hospice & Palliative Care in North Georgia seeks a Nurse Case Manager (RNCM) to plan and direct hospice care using the nursing process. You will collaborate with the Interdisciplinary Team to support patients and families with compassion and professional nursing care.

We require a Georgia RN license, 1–2 years of nursing experience (hospice preferred), a valid driver's license and reliable vehicle.

Qualifications

Current RN license in the State of Georgia.
1–2 years of nursing experience (preferred).
1–2 years of hospice experience preferred (candidates with experience in skilled nursing, home health, acute care, med-surg, or ICU will be considered).
Utmost compassion and integrity.
Valid driver’s license, insurance, and reliable vehicle.
Excellent time management skills.

Responsibilities

Create, plan, and direct hospice care using the nursing process.
Collaborate with Interdisciplinary Team and build relationships with patients’ families.
Ensure highest standards of care and professional nursing competency.
