Job summary

Tapestry Hospice is seeking a Nurse Case Manager (RNCM) in North Georgia to join a dedicated care family. You will lead hospice care planning and coordinate with the interdisciplinary team to ensure compassionate, high-quality patient care.

Ideal candidates hold a Georgia RN license with 1–2 years of nursing and hospice experience, strong time management, and a reliable vehicle to support visiting patients across communities. We offer competitive compensation and comprehensive benefits.

Qualifications

Current RN license in the State of Georgia.
1–2 years of nursing experience (preferred).
1–2 years of hospice experience preferred (or related settings allowed).
Utmost compassion and integrity.
Valid driver’s license and reliable vehicle.
Excellent time management skills.

Responsibilities

Create, plan, and direct hospice care using the nursing process of assessment, intervention, implementation, and evaluation.
Collaborate with the Interdisciplinary Team to support patients and families.
Maintain professional nursing standards and clinical competency.
