Tapestry Hospice & Palliative Care in North Georgia is seeking a Nurse Case Manager (RNCM) to lead hospice care using the nursing process. You will work with an Interdisciplinary Team to coordinate patient and family needs with compassion and clinical excellence.

As a Nurse Case Manager (RNCM), you will create, plan, and direct hospice care by utilizing the nursing process of assessment, intervention, implementation, and evaluation. You will collaborate closely with all members of the Interdisciplinary Team while building meaningful relationships with patients and their families. This role requires a commitment to the highest standards of care and professional nursing clinical competency.
