Job summary

Tapestry Hospice & Palliative Care in North Georgia is seeking a Nurse Case Manager (RNCM) to lead hospice care using the nursing process. You will work with an Interdisciplinary Team to coordinate patient and family needs with compassion and clinical excellence.

The ideal candidate holds a Georgia RN license with 1–2 years of nursing and hospice experience, a valid driver’s license, and strong time management. Join a family-owned team dedicated to care and respect.

Qualifications

Current RN license in the State of Georgia.
1–2 years nursing experience preferred.
1–2 years hospice experience preferred (candidates with other nursing backgrounds considered).
Utmost compassion and integrity.
Valid driver’s license, insurance, and reliable vehicle.
Excellent time management skills.

Responsibilities

Create, plan, and direct hospice care by utilizing the nursing process (assessment, intervention, implementation, evaluation).
Collaborate closely with the Interdisciplinary Team.
Build meaningful relationships with patients and their families.
