Job summary

Tapestry Hospice & Palliative Care in Georgia seeks a Nurse Case Manager (RNCM) to coordinate and oversee hospice care using the nursing process. You will collaborate with physicians, nurses, social workers, and the interdisciplinary team to ensure compassionate, patient-centered care.

The ideal candidate holds a GA RN license with 1–2 years of nursing and hospice experience, strong time management, and a valid driver's license.

Qualifications

• Current GA RN license is required.
• 1–2 years nursing experience preferred.
• 1–2 years hospice experience preferred; other acute care backgrounds considered.
• Utmost compassion and integrity.
• Valid driver’s license, insurance, and a reliable vehicle.

Responsibilities

• Create, plan, and direct hospice care using the nursing process.
• Collaborate with the Interdisciplinary Team to support patients and families.
• Build meaningful relationships with patients and families while delivering high-quality care.
• Uphold professional nursing standards and judgment in care delivery.
