Serenity Hospice Care in New Jersey is seeking a dedicated RN Case Manager to deliver compassionate hospice care, assess patient needs and develop individualized care plans.
You will collaborate with physicians, nurses, and family members to ensure coordinated care, perform regular checks, and support end-of-life decisions.
A NJ RN license and hospice experience are required; willingness to travel within Mercer County and maintain reliable transportation.

Serenity Hospice is looking to add a dedicated and compassionate RN Case Manager to delivering exceptional care, comfort and support to patients. They will develop plans for individual needs, perform regular check-ups and communicate with family members as needed. This position will oversee Mercer County.
Complete an initial assessment of patient and family to determine hospice care needs
Regularly re-evaluates the patients nursing needs
Use assessment data to determine care plan needs
Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide care that is continuous and well-coordinated
RN counsels the patient and family in meeting end of life needs
Registered Nurse initiates appropriate preventative nursing procedures
Communicates with the physician regarding patient's needs and reports any changes in the patient's condition
Supervises the Hospice Aides and LPN's
Current NJ RN License in good standing
One year Hospice Experience REQUIRED
One year of Hospital Setting Experience REQUIRED
Willing to travel to the assigned areas
Must have reliable transportation with liability insurance
Strong communication and interpersonal skills.
