Job summary
Serenity Hospice Care in New Jersey is seeking a dedicated RN Case Manager to deliver compassionate hospice care, assess patient needs and develop individualized care plans.
You will collaborate with physicians, nurses, and family members to ensure coordinated care, perform regular checks, and support end-of-life decisions.
A NJ RN license and hospice experience are required; willingness to travel within Mercer County and maintain reliable transportation.
Qualifications
- Current NJ RN License in good standing.
- One year Hospice experience required.
- One year hospital setting experience required.
- Willing to travel to assigned areas.
- Must have reliable transportation with liability insurance.
- Strong communication and interpersonal skills.
Responsibilities
- Complete initial assessment of patient and family to determine hospice needs.
- Regularly re-evaluates the patient’s nursing needs.
- Use assessment data to determine care plan needs.
- Collaborate with the health team and family to coordinate care.
- Counsel patient and family on end-of-life needs.
- Initiate appropriate preventative nursing procedures.
- Communicate with physician about patient needs and changes.
- Supervise Hospice Aides and LPNs.
