Arkansas Hospice, Inc. is seeking a Registered Nurse Case Manager to oversee clinical interventions for an assigned hospice caseload.

The role involves coordinating care plans, collaborating with an interdisciplinary team, and ensuring timely documentation and communication with patients, families, and physicians. The ideal candidate is an Arkansas-licensed RN, with BSN preferred and at least one year of nursing experience in hospice or related settings.

Qualifications

• Active RN license in Arkansas.
• BSN preferred.
• Minimum one year nursing experience in hospital, home health, hospice, or related program.

Responsabilités

• Manage patient care after admission and assignment to designated RN Case Manager.
• Assist in admission process as requested.
• Explain and apply Arkansas Hospice, Inc. policies to nursing staff and others.
• Coordinate and collaborate with interdisciplinary team for care plans.
• Update Plan of Care as patient status changes.
• Communicate with patients, families, and healthcare providers.
• Maintain contact with attending physician and community resources.
• Monitor pain and symptom control for patient comfort.
• Coordinate CNA scheduling with Patient Care Coordinator/Area Manager.
• Initiate referrals for hospice services.
• Act as preceptor for nurses, students, and trainees.
• Review staff documentation for completeness and accuracy.
