Best Hospice Care of Texas in Beaumont, TX is seeking a compassionate Full-Time RN Case Manager to join our close-knit team and provide individualized hospice care to patients and families.

You will manage a full caseload, perform skilled nursing visits, develop care plans, educate families, and collaborate with physicians and the interdisciplinary team, ensuring timely documentation and continuity of care.

Qualifications

• Current Texas RN license.
• Compassionate, dependable, and organized.
• Strong clinical assessment and communication skills.
• Ability to independently manage and prioritize a full caseload.
• Hospice or home health experience preferred.

Responsibilities
