Best Hospice Care of Texas is seeking a Full-Time RN Case Manager to join our compassionate team in Texas. You will manage a full patient caseload, perform skilled nursing visits, and develop individualized plans of care with patients, families, and the interdisciplinary team.

This role emphasizes compassionate, organized nursing, strong communication, and timely documentation, with participation in IDT meetings and on-call rotation as assigned.

Qualifications

• Current Texas RN license.
• Compassionate, dependable, and organized.
• Strong clinical assessment and communication skills.
• Ability to independently manage and prioritize a full caseload.
• Hospice or home health experience preferred.
