Job summary

Bristol Hospice in Massachusetts is seeking RN Case Managers to advocate for patients and families with compassionate, whole-person care. You will develop individualized care plans, monitor vitals, educate families, and coordinate with an interdisciplinary team to ensure comfort and dignity throughout the journey.

A valid MA RN license, CPR certification, and a valid driver’s license are required. Hospice or home health experience is preferred.

Qualifications

• Must hold a valid RN license in the state of practice.
• CPR certification current and active.
• Strong communication with patients, families, and teams.
• Experience in hospice or home health care is preferred.

Responsibilities

• Develop and implement individualized care plans based on medical history.
• Deliver one-on-one support to patients and families, ensuring comfort and dignity.
• Monitor vital signs and document changes accurately in records.
• Administer physician-ordered meds and treatments with empathy.
• Educate families on care techniques and symptom management.
• Collaborate with interdisciplinary teams for comprehensive care.
