Job summary

Bristol Hospice is seeking an RN Case Manager to provide comprehensive, compassionate care for patients and families in Oklahoma, including Fort Gibson. You will develop individualized plans, monitor clinical needs, and partner with a multidisciplinary team to ensure comfort, dignity, and support throughout the hospice journey.

Qualifications

• Valid RN license in the state you will work in.
• CPR certification current.
• Strong observational and written/verbal communication skills.
• Valid driver’s license and reliable transportation.
• Willingness to take on additional duties as needed.
• Commitment to compassionate end-of-life care.

Responsibilities

• Develop and implement individualized care plans based on current and prior medical history.
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding.
• Monitor vital signs and document accurately.
• Administer physician-ordered medications and treatments with skill and empathy.
• Educate patients and families on care techniques and symptom management.
• Collaborate with interdisciplinary teams for comprehensive care delivery.
