Job summary

Frontier Hospice is seeking a dedicated Full-Time Hospice RN Case Manager to deliver compassionate end-of-life nursing care in Norman, Moore, and Midwest City. You will coordinate services and guide patients and families through every step of the hospice journey.

As part of our interdisciplinary team, you’ll work with the Clinical Director to assess eligibility, tailor care plans, educate families, and respond to referrals with empathy and expertise.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.

Responsibilities

• Collaborative Care Champion: Partner closely with our Clinical Director to assess the eligibility and suitability of clients for our specialized hospice services.
• Nurturing Direct Care: As an RN Case Manager, you're coordinating care to ensure comfort and quality of life remain paramount.
• Nursing with Heart: Deliver top-notch care rooted in proven principles and techniques to ease pain and bring solace.
• Educator and Advocate: Educate patients and families to participate in creating personalized care plans.
• Call of Compassion: Participate in call duties to be there when it matters most.
