Bristol Hospice is hiring a Registered Nurse Case Manager to provide compassionate, whole-person care and advocacy for patients and families in Alabama. You will develop individualized care plans, monitor symptoms, administer treatments, and educate families while collaborating with a multidisciplinary team in a home-like environment.

Qualifications

• A valid Registered Nurse license in good standing in the state you will work in.
• CPR certification current and up to date.
• Hospice or palliative care experience preferred.

Responsibilities

• Develop and implement individualized care plans based on medical history.
• Deliver attentive, one-on-one support to patients and families, ensuring comfort and dignity.
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey.
• Monitor vital signs and document changes; notify physicians as needed.
• Administer physician-ordered medications and treatments with skill and empathy.
• Educate patients and families on care techniques and symptom management.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
• Document patient progress and quality of care with detailed clinical notes.
• Support additional duties as assigned within a mission-driven hospice team.
