Job summary

Interim HealthCare Hospice & Palliative Care in Defiance, OH is seeking a compassionate Hospice Registered Nurse Case Manager (RN) to coordinate patient care across the care team and ensure dignity and comfort for clients.

You will assess, update, and implement individualized care plans, collaborate with spiritual care coordinators, social workers, aides, and medical directors, and support families during challenging times.

Qualifications

• Education: ADN or AAS required; BSN preferred.
• Licensure: Current RN license in the state.
• Current CPR/FA certification.
• Reliable transportation to care sites and/or work locations.
• One year of RN experience in hospice or similar setting preferred.

Responsibilities

• Oversee the implementation and ongoing assessment of the patient's plan of care alongside the spiritual care coordinator, social worker, hospice aide and medical director.
• Develop and implement a comprehensive care plan that addresses patient needs.
• Communicate patient conditions and collaborate with appropriate providers to deliver care when patient needs evolve.
