Job summary

Agape Hospice NW in Clackamas, Oregon, is seeking a Registered Nurse Case Manager to join our hospice team. This full-time role coordinates home-based care, admissions, assessments, care planning, and collaboration with the interdisciplinary team to ensure high-quality end-of-life care.

Requires an active RN license, 2+ years nursing experience (hospice/home health preferred), CPR, a valid driver’s license and vehicle.

Qualifications

• Graduate of an accredited nursing program.
• Current, unrestricted RN license in good standing.
• Minimum of two (2) years of nursing experience; hospice, home health, or community health experience preferred.
• Current CPR certification.
• Valid driver’s license and access to an insured, reliable vehicle.
• Strong assessment, critical‑thinking, communication, and organizational skills.
• Ability to work independently, prioritize workload, and manage time effectively.
• Knowledge of hospice philosophy, interdisciplinary care models, and end‑of‑life practices.

Responsibilities

• Conduct comprehensive assessments of patients and families to identify hospice needs.
• Perform hospice admissions and start‑of‑care visits with comprehensive assessments and documentation.
• Develop, implement, and regularly update individualized plans of care.
• Serve as the primary Case Manager for assigned patients and coordinate services across the team.
• Administer medications and evaluate patient response to interventions.
• Provide ongoing pain and symptom management and collaborate with the team.
• Educate patients and families about disease progression, medications, and care techniques.
• Maintain accurate clinical documentation reflecting patient status and outcomes.
• Communicate changes in patient condition promptly to physicians and team members.
• Participate in on‑call rotation and ensure continuity of care.
