Job summary

Phoenix Home Care and Hospice in Missouri is seeking a Registered Nurse Case Manager to provide compassionate nursing care to clients and families facing a terminal diagnosis. You will collaborate with the interdisciplinary team to develop and implement a patient/family centered plan of care, including goals of care discussions and guidance on healthcare decisions to improve quality of life.

Qualifications

• Missouri RN license or Compact RN license required.
• Valid driver's license and auto liability insurance.
• Missouri Compact license accepted where applicable.

Responsibilities

• Educate patients and families about the process, plan of care, and healthcare decisions guidance.
• Consult with attending physician and Hospice Medical Director regarding changes to patient condition or plan of care.
• Coordinate hospice care with the team and families to ensure needs are met.
• Conduct initial and ongoing comprehensive assessments.
• Perform hospice care evaluations and informational visits.
