Job summary

Gentiva in Dayton, OH is seeking a compassionate Hospice RN / RN Case Manager to lead patient care in home settings. You will assess, plan, implement, and document skilled nursing services aligned with the patient’s Plan of Care, coordinating with the IDG and collaborating with physicians.

The role emphasizes family education, symptom management, and bereavement support, with on-call responsibilities and ongoing quality improvement.

Qualifications

• RN license in the state of employment.
• Minimum 1 year experience as an RN in hospice, home health, oncology, or related settings.
• Hospice or palliative care experience preferred.
• CPR certification.
• Current automobile insurance and valid driver’s license.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings with patient and family education.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized care plans including visit frequency and risk identification.
• Coordinate care with IDG, physicians, and other healthcare professionals.
• Provide education to patients and caregivers on disease process, end‑of‑life care, medications, nutrition, and self‑care.
