Job summary
Gentiva Heartland Hospice is seeking a dedicated Registered Nurse (RN) to provide comprehensive hospice care in patients' homes and community settings. You will lead skilled nursing interventions, conduct assessments, and develop personalized plans of care in collaboration with an interdisciplinary team.
Strong communication with patients, families, and colleagues is essential. The role emphasizes compassionate, patient-centered care, with opportunities to educate family caregivers and support
Qualifications
- Active RN license in the state of employment.
- Hospice or palliative care experience preferred.
- CPR certification.
- Valid driver’s license and auto insurance.
Responsibilities
- Perform comprehensive patient assessments and reassessments, including in-person/telephonic evaluations.
- Deliver skilled nursing interventions and symptom management per care goals.
- Develop and update individualized plans of care and visit frequency.
- Coordinate care across IDT, including physicians and other clinicians.
- Educate patients and family caregivers on disease, self-care, medications, nutrition, and end-of-life care.
