A leading hospice care provider in Michigan is seeking a skilled RN Case Manager to provide professional nursing care to patients, families, and caregivers. The role requires a valid RN license from the State of Michigan and experience in hospice or critical care settings. The RN will supervise aides, assess patient needs, and ensure a comprehensive Plan of Care is created and followed. This position emphasizes high-quality symptom management and compassionate support for the terminally ill.

Qualifications

• Graduate of an accredited school of professional nursing. BSN preferred.
• Maintains license as a Registered Nurse from the State of Michigan.
• At least one year of general nursing experience required.
• Current BLS certification required.

Responsibilities

• Provide skilled professional nursing care to patients/families/caregivers as prescribed.
• Supervise Hospice Aides and coordinate care.
• Assess and evaluate patient status; write and revise Plan of Care.
