Job summary

Suncoast Hospice of Hillsborough, part of Empath Health, seeks an RN Case Manager to join the team. This field-based role provides direct patient care in private homes and facilities across South Tampa, weekdays during daytime hours.

Build compassionate plans of care with the IDT, guide families through the hospice journey, and supervise field staff as needed. The ideal candidate has an active Florida RN license and at least one year of case management or home health experience; bilingual

Qualifications

• Registered nurse with valid Florida license.
• Minimum of one year of case management or home health experience.
• Bilingual Spanish/English preferred.

Responsibilities

• Assess patients and families; develop a Plan of Care with patient, family, IDT and attending physician.
• Coordinate care and supervise LPNs and Home Health Aides per policy.
• Administer and document medications and provide patient/family teaching.
