Job summary
Suncrest Hospice is seeking a registered nurse to provide direct patient care in home settings, Skilled Nursing Facilities, and assisted living facilities. You will be part of an interdisciplinary team addressing psych-social needs and coordinating patient care across case management, admissions, or resource roles.
The role requires an active RN license, strong teamwork, and the ability to travel across the service area using your own vehicle.
Qualifications
- Current Registered Nurse (RN) licensure in good standing
- Works well within a team environment with other healthcare professionals
- Ability to work independently with phone, email, and visits
- Strong time management to schedule visits and prioritize needs
- Ability to learn an electronic medical records system
- Willingness to drive across the service area with reliable transportation
Responsibilities
- Provide care and services for a group of patients in homes, SNFs, or assisted living facilities
- Visiting patients where they reside and coordinating with the care team
- Contribute as part of the interdisciplinary team in case management, admissions, or resource roles
