Job summary

Trustbridge Hospice, part of Empath Health, is seeking a compassionate RN Hospice Case Manager in the Boca area to coordinate patient-centered end-of-life care Monday–Friday with weekend rotation.

You will advocate for patients, coordinate care with the IDT, develop individualized plans, monitor conditions, provide education, and support families.

Florida RN license, BLS, and hospice or related nursing experience preferred; EMR skills and reliable transportation required.

Qualifications

• Must hold Florida RN license.
• Current BLS certification.
• Experience in hospice or related nursing preferred.
• Strong case management and communication skills.

Responsibilities

• Advocate for patients and families.
• Coordinate care under physician and IDT.
• Develop and implement individualized care plans (IDG).
• Monitor patient conditions and provide interventions.
• Deliver education to enhance comfort and quality of life.
• Address concerns to maintain satisfaction.
