Job summary

Conemaugh Regional Hospice-Johnstown, part of the Optum family, is seeking a Registered Nurse Case Manager to oversee hospice care and carry out the Plan of Care for patients. You will collaborate with the inter-disciplinary group to implement strategies that promote physical, psychosocial, emotional, and spiritual well-being.

The role requires an active RN license, a valid driver's license, and the ability to travel to patient homes.

Qualifications

• RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• 1+ years of clinical experience.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Make initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Assess patient/family physical, psychosocial, and environmental needs regularly (every 15 days).
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions, and responses to care.
• Collaborate with patient/family and care team to provide daily care.
• Instruct patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes on patient condition.
• Provide care across diverse home settings regardless of client background.
