Job summary

UnitedHealth Group is seeking a Hospice Registered Nurse (RN) to deliver compassionate, patient-centered care in home-based hospice settings. You will assess needs, develop and implement a Plan of Care, and coordinate care with the interprofessional team to support physical, psychosocial, and spiritual well-being for patients and families.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• 1+ years of full-time experience as an RN; substitution: three years of full-time clinical experience as an LPN may substitute for RN experience.

Responsibilities

• Makes the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs; reassesses at least every 15 days.
• Initiates and coordinates the plan of care; documents goals, interventions, and patient/family response.
• Collaborates with patient/family, attending physician, and IDG for daily care.
• Instructs and supervises patient/family in self-care as appropriate.
• Maintains accurate clinical notes.
• Responds to any home situation irrespective of patient demographics.
