Job summary

Heart to Heart Hospice is seeking an RN Case Manager in Lafayette, IN to deliver comprehensive, patient-centered care for individuals with life-limiting illnesses. You will assess physical, psychosocial, and environmental needs, coordinate care with families and the IDG, and adapt plans as conditions change.

The role emphasizes compassionate service, adherence to regulations, and collaboration with a multidisciplinary team to improve quality of life for patients and families.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License, reliable transportation, and provide ongoing valid auto liability insurance.
• Two years of RN experience preferred in a healthcare/clinical setting.

Responsibilities

• Initial and ongoing comprehensive assessments of the patient and family needs and ADLs.
• Implement the individualized Plan of Care and revise as necessary.
• Collaborates care among patient, caregiver, physicians and health professionals.
• Attend IDG meetings and coordinate patient care planning with the team.
• Perform on-call responsibilities and provide on-call service as assigned.
• Timely submission of all documentation.
• Ensure compliance with laws, regulations, and policies.
