Miracle City Hospice in Ashtabula, OH seeks a compassionate Hospice CNA/STNA to support patients at home, following individualized care plans and maintaining patient dignity. Schedule is Monday–Friday, 8:00 AM – 5:00 PM, with collaborative support from our hospice team.

Qualifications

• Current Ohio CNA or STNA certification.
• Current CPR certification.
• Active status on the Ohio aide registry.
• Ability to work independently within a care team.
• Strong communication and organizational skills.
• Valid driver’s license with reliable transportation.
• Compassionate, empathetic, patient-focused approach.

Responsabilités

• Provide personal care and assist with ADLs (bathing, grooming, dressing, oral hygiene).
• Support patient comfort and safety by following care plans.
• Assist with mobility, transfers, ambulation, and prescribed exercises.
• Monitor and report changes to the RN Case Manager.
• Record vital signs and document care accurately.
• Assist with meal preparation, feeding, and light tasks.
• Provide emotional support and companionship to patients and families.
• Maintain a clean, safe, respectful home environment.
• Offer respite support to caregivers when appropriate.
• Collaborate with hospice team to ensure exceptional care.
