Job summary

Bristol Hospice Northglenn is seeking an RN Case Manager to provide compassionate, whole-person care and advocate for patients and families. You will develop and implement individualized care plans, monitor progress, and coordinate with interdisciplinary teams to ensure comfort and dignity throughout the care journey.

The role requires an active RN license in good standing, CPR certification, a valid driver’s license, and strong communication skills.

Qualifications

• Active RN license in good standing in state of practice.
• CPR certification current and valid.
• Strong observational and communication skills, written and verbal.
• Willingness to travel for patient visits as needed.

Responsibilities

• Deliver compassionate, one-on-one care to patients and families.
• Develop and implement individualized care plans.
• Monitor vital signs and document progress accurately.
• Administer physician-ordered medications and treatments.
• Educate patients and families on care techniques and symptom management.
• Collaborate with interdisciplinary teams to ensure comprehensive care.
