Job summary

Bristol Hospice is seeking an RN Case Manager to provide compassionate, whole-person care for patients and families in the Altoona, Tyrone, and Cambria territory. This full-time role requires an active PA RN license, CPR certification, a valid driver’s license, and strong observational and communication skills.

You will develop individualized care plans, monitor vitals, educate families, and collaborate with a diverse care team.

Qualifications

• Valid PA RN license in good standing.
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• Excellent observational and communication skills.
• Willingness to support additional duties as needed.

Responsibilities

• Develop and implement individualized care plans based on current and prior medical history.
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding.
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey.
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure.
• Administer physician-ordered medications and treatments with skill and empathy.
• Educate patients and their families on care techniques and symptom management.
• Track and communicate changes in patient condition, ensuring timely and effective responses.
• Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
• And additional duties as assigned.
