Job summary

VitalCaring is seeking an RN for hospice care leadership. You will coordinate care across an interdisciplinary team, guide physicians and families, and ensure plans of care reflect patient goals with comfort and dignity.

Requirements include an active RN license, hospice/home health experience, and 2+ years in leadership or care coordination. This is a full-time role with comprehensive benefits and growth opportunities.

Qualifications

• Active RN license in state of practice.
• BSN preferred; nursing degree from an accredited program.
• Hospice or Home Health experience required.
• 2+ years leadership or care coordination experience.
• Strong communication and teamwork abilities.
• Valid driver’s license and CPR certification.

Responsibilities

• Coordinate and oversee hospice patient care across the interdisciplinary team.
• Serve as the central point of communication between physicians, patients, families and internal teams.
• Manage physician orders and ensure timely entry, approval and alignment with the plan of care.
• Lead development and updates of individualized hospice plans of care.
• Support patients and families through education, symptom management, and end-of-life planning.
• Review clinical reports and participate in IDG meetings; support care planning.
