Job summary

Continuum Hospice is seeking a compassionate Registered Nurse Case Manager (RNCM) to join our dedicated nursing team. The role covers Santa Cruz and San Jose, focusing on end-of-life care with a patient-centered approach.

As an RNCM, you will coordinate care, educate families, and collaborate with leadership to ensure timely, quality hospice services. Excellent communication and CPR certification are required; prior hospice experience is a plus.

Qualifications

Hold current unencumbered license as a Registered Nurse.
Bachelor's degree in nursing from a program accredited by the NLN.
One year nursing experience within the last three years.
Hospice/home health experience preferred but not required.
Proficient with computers, typing, and EMR systems.
Current CPR certification.
Valid driver's license, vehicle insurance, and reliable transportation.

Responsibilities

Empower through engagement: respond to clinical referrals with care and expertise.
Collaborate with Clinical Director to assess client eligibility for hospice services.
Provide direct care coordination to ensure comfort and quality of life.
Deliver nursing care rooted in proven principles to ease pain and distress.
Educate and advocate for patients and families to participate in care plans.
Share call duties to be there when it matters most.
