Job summary

Continuum Care Hospice seeks a compassionate Registered Nurse Case Manager (RNCM) to join our team in Alameda and Contra Costa Counties. This full-time role involves hands-on hospice nursing, coordinating services, and guiding patients and families through their hospice journey with empathy and clinical excellence.

The RNCM will collaborate closely with the Clinical Director, deliver individualized care plans, and participate in on-call coverage.

Qualifications

• RN license required (unencumbered).
• 1 year of nursing experience in the last 3 years.
• At least 1 year of hospice experience.
• Proficient in computer use and EMR systems.
• Current CPR certification required.
• Valid driver’s license, vehicle insurance, and reliable transportation.

Responsibilities

• Provide hands-on hospice nursing care and coordinate services.
• Collaborate with Clinical Director to assess client eligibility and needs.
• Deliver compassionate care and maintain patient comfort and quality of life.
• Educate and engage patients and families in care planning.
• Participate in on-call duties as part of the team.
