Job summary

Continuum Hospice is seeking a Full Time Hospice Registered Nurse Case Manager (RNCM) to join our nursing team providing end-of-life care across the Santa Cruz and San Jose communities.

In this role you will deliver hands-on nursing care, coordinate services, and guide patients and families through the hospice journey, with a Monday–Friday schedule and a supportive onboarding program and comprehensive benefits.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Bachelor's degree in nursing (BSN) or equivalent.
• One (1) year experience as a professional nurse within the last three (3) years.

Responsibilities

• Deliver hands-on hospice nursing care.
• Coordinate services and guide patients and families.
• Collaborate with the interdisciplinary care team.
• Educate patients and families on care plans.

Continuum Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

Schedule: Monday - Friday, 8 am - 4:30 pm

Territory: Santa Cruz and San Jose

• Collaborative Care Champion: Partner closely with our Clinical Director to assess the eligibility and suitability of clients for our specialized hospice services. Your expertise will guide us in providing the right care, at the right time.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Bachelor's degree in nursing from a program accredited by the National League for Nursing; OR
  • One (1) year experience as a professional nurse within the last three (3) years.
• Hospice/home health experience preferred but not required.
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification.
• Current driver's license, vehicle insurance, and access to dependable transportation.

$64.00 - $66.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate's relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
