Job summary
Altus Hospice Care is seeking a Full Time Hospice RN Case Manager to join our nursing team in Marble Falls, providing compassionate end-of-life care and coordinating services across the patient journey. You will deliver hands-on nursing care, support patients and families, and collaborate with an interdisciplinary team to ensure the best care possible.
Active RN license and at least 1 year of medical-surgical experience are required, with EMR proficiency and CPR as applicable.
Qualifications
- Active RN license in good standing.
- Minimum 1 year RN experience in medical-surgical or acute care; hospice a plus.
- Proficient with electronic medical records (EMR).
- CPR certification current if required by state.
- Reliable transportation with valid driver's license and insurance.
Responsibilities
- Coordinate hospice services and support patients and families.
- Respond to referrals and assess eligibility for services.
- Deliver hands-on nursing care and monitor patient comfort.
- Educate families and empower care planning.
- Participate in call duties as needed.
