Job summary
Altus Hospice is seeking a Full Time Hospice Registered Nurse Case Manager (RNCM) to join our dedicated nursing team throughout the Southeast Houston area. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey.
This role reports to the Clinical Director with a Monday–Friday 8 am–4:30 pm schedule and on-call included. We offer a comprehensive benefits package, PTO, holidays, tuition assistance, wellness
Qualifications
- Current unencumbered RN license.
- Minimum 1 year RN experience in medical-surgical/acute care; hospice a plus.
- Proficient computer skills and EMR experience.
- CPR certification (as required by state).
- Valid driver's license and reliable transportation.
Responsibilities
- Provide hands-on hospice nursing care in patients' homes or facilities.
- Coordinate services and communicate with interdisciplinary care team.
- Guide patients and families through the hospice journey with compassion.
- Deliver care that focuses on comfort, quality of life, and symptom management.
- Educate and empower patients and families to participate in care planning.
- Share call duties as part of the team.
