Job summary

Altus Hospice Care is seeking a Full Time Hospice RN Case Manager to join our nursing team in Marble Falls, providing compassionate end-of-life care and coordinating services across the patient journey. You will deliver hands-on nursing care, support patients and families, and collaborate with an interdisciplinary team to ensure the best care possible.

Active RN license and at least 1 year of medical-surgical experience are required, with EMR proficiency and CPR as applicable.

Qualifications

• Active RN license in good standing.
• Minimum 1 year RN experience in medical-surgical or acute care; hospice a plus.
• Proficient with electronic medical records (EMR).
• CPR certification current if required by state.
• Reliable transportation with valid driver's license and insurance.

Responsibilities

• Coordinate hospice services and support patients and families.
• Respond to referrals and assess eligibility for services.
• Deliver hands-on nursing care and monitor patient comfort.
• Educate families and empower care planning.
• Participate in call duties as needed.

Altus Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

• Collaborative Care Champion: Partner closely with our Clinical Director to assess the eligibility and suitability of clients for our specialized hospice services. Your expertise will guide us in providing the right care, at the right time.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

$76,500 - $87,500 per year (Average Pay Range)

Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
