Job summary
Altus Hospice is seeking a compassionate Full Time Hospice RN Case Manager to join our nursing team delivering end-of-life care in Victoria and surrounding areas. Schedule is Monday–Friday, 8:00 am to 4:30 pm, with responsibilities spanning direct patient care, care coordination, and family guidance.
The RN will collaborate with the Clinical Director and interdisciplinary team to ensure dignity, comfort, and high-quality service.
Qualifications
- Hold current unencumbered RN license.
- Minimum 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
- Computer proficiency in typing and various programs; EMR background.
- Current CPR certification if state requires.
- Valid driver's license, vehicle insurance, and reliable transportation.
Responsibilities
- Empower Through Engagement: respond to referrals with care and expertise.
- Collaborative Care Champion: work with Clinical Director to assess client eligibility for hospice services.
- Nurturing Direct Care: deliver hands-on hospice nursing and ensure patient comfort and quality of life.
- Nursing with Heart: provide care grounded in proven nursing principles.
- Educator and Advocate: educate patients and families to participate in care plans.
- Call of Compassion: participate in on-call duties as part of the team.
